Transfer of community services

From 1 July, the health services you currently receive in the community, are becoming part of Bolton NHS Foundation Trust, which also includes services at the Royal Bolton Hospital.

 

These services include:

 

  • district nurses
  • health visitors
  • School nurses
  • services provided at health centres and clinics.

Responsibility for the records of your care in the community will be transferred to Bolton NHS Foundation Trust and will remain secure and confidential. This does not affect the records your GP has.

 

Access to your information is strictly controlled and only given to those who need to know. You have a right to privacy. Everyone working in the NHS has a legal duty to keep information about you confidential. 

 

We will only pass on information about you if it is needed for your treatment and care and is in your and everyone’s interest.
