Summary
The purpose of this strategy is to provide NHS Bolton with an authoritative, evidence based comprehensive approach to addressing Long Term Conditions. Long-term Conditions are conditions that cannot, at present, be cured, however can be controlled by medication and other therapies. They include diabetes, asthma, chronic obstructive pulmonary disease, severe mental health, epilepsy, coronary heart disease and many others.
The paper sets out the context of addressing Long Term Conditions from National Government policy documents, academic reports and Strategic Health Authority work, utilising their recommendations and models of care.
