The Palliative Care Therapy Team

 

Who we are

We are a joint Physiotherapy and Occupational Therapy Team based at Bolton Hospice.  Our team has recently been expanded with funding from the Bolton End of Life Strategy to provide care to all patients with end of life needs.  We see both Hospice and Community patients with any diagnosis, including non-malignant conditions, when it seems likely that they are in the last year of their life. 

 

How the Team works

The overall aim of our service is to offer palliative and supportive care to any patient with end of life needs, supporting the Gold Standards Framework, LCP and Preferred Priorities of Care.  Our aim is to help patients adapt to their condition, retain function and independence where possible, help with symptom control, including self management techniques, and to plan for future changes.

 

What do we offer?

The treatment programmes involve provision of equipment and advice to maintain independence and function with particular reference to anticipating future deterioration.  We also provide functional exercise programs and symptom control, particularly with respect to breathlessness, fatigue, weakness and anxiety.  We can offer the loan of TENS machines, wheelchair and seating assessments, cognitive assessments and simple lymphoedema management.  We see patients in their own homes or at the Hospice if they attend as inpatients, outpatients or day therapy patients

Support to Professionals

The Team also offers support and training for Health Care Professionals who work with any patients with end of life care needs.  We can give information and advice over the phone and are happy to discuss any cases to identify whether they are suitable for our service.  We are also happy to meet other teams to discuss our service and how we can work with other teams to provide the best patient care.
