What does the service provide?

  • Assessment and coordination of services that facilitate independence and optimum health for patients who suffer from chronic disease, and who are regular users of primary and secondary health care services.
  • Coordination of services for all patients who suffer from a chronic disease.
  • A reduction in the number of admissions to secondary care for patients with chronic diseases - through offering support, advice and coordinating appropriate care provision in the community.
  • Full clinical medication review with the patient, liaising with GP and other members of the Multi-Disciplinary Team.

Who is the service for?

Any person registered with a GP within the boundaries of NHS Bolton.

Patients who are identified with a chronic disease, and who are high intensity users of health services i.e. GP visits, attendances at A&E and frequent hospital admissions.

NHS Bolton delivers Active Case Management via Specialist Teams. 

The teams have specialist interests in:

·         Falls

·         Stroke

·         Long Term Neurological Conditions

·         Children

All are supported by the Active Case Management Pharmacist.

When is the service available?

Monday to Friday 8.00 am to 5 pm.

Any time out of these hours patients would need to contact their local out of hours service or NHS Direct 0845 46 47.

How can people access the service?

Referrals are received from the following:

  • Royal Bolton Hospital, or any other acute hospitals in Manchester.
  • GPs.
  • District Nurses.
  • Any appropriate health care professional who believes that a patient may benefit from the service.

Each case manager has specific criteria developed from the PARR Predictive Tool and current Government guidance.
