How the Team works

The overall aim of our service is to provide a safe and effective monitoring and advice service for any Bolton PCT patients referred to us on oral Anticoagulant therapy. We are a nurse led service with adhoc support and advice provided by a Haematologist from Royal Bolton Hospital.

 

What do we offer?

The treatment programmes involve a comprehensive Anticoagulant introduction and drug counseling, monitoring and dose adjustment plus ad hoc advice to patients, carers and other professionals on matters relating to Anticoagulant therapy.

 

Support to Professionals

The Anticoagulant Team also offers support and training for Health Care Professionals who work with patients on Anticoagulant therapy.

 

How to access the service

A fully completed referral from a GP or hospital doctor is required before an appointment can be made. Once this is received appointments can be made.

  • Great Lever Health Centre – Tuesday AM
  • Little Lever Health Centre – Tuesday PM
  • Horwich Clinic – Wednesday AM
  • Westhoughton Clinic – Wednesday PM
  • Tonge Moor Health Centre – Thursday AM
  • Avondale Health Centre – Thursday PM
  • Farnworth Health Centre – Friday AM
  • Pikes Lane Centre – Friday AM
