Who we are

 

Bolton Community Unit opened in July 2006 and is situated on the main hospital corridor adjacent to A&E at the Royal Bolton Hospital. We are an integrated team consisting of Health and Social Care professionals who work together to try and keep people out of hospital whenever it is safe to do so. The team has knowledge and familiarity of community services, providing the greatest opportunity for success in identifying patients whose care could be provided outside of hospital. The team opened, initially operating a 16 hour day but due to demand quickly expanded to 24/7.

 

 

How the Team works

 

The overall aim of our service is to avoid hospital admissions whenever it is safe to do so. Patients arriving in A&E are either identified and referred by the A&E team or identified and pulled through by the Community Unit team who regularly attend A&E and select patients based on clearly defined criteria. The main requirement prior to transfer is a planned exit strategy.  

 

 

What do we offer?

 

The treatment and care programmes involve support from step up services and facilities that support hospital admission avoidance such as Residential and Domiciliary Intermediate Care, Crisis response and Hospital at Home. Nearly 80% of patients are discharged back home often with a package of care or treatment plan following a holistic multidisciplinary assessment. There is access to a competent professional consistently across a 24 hour period.

    

How to access the service

 

Patients are referred directly from A&E or the referral and assessment team. A small number of patients arrive directly for planned treatment or as referrals from other community services requiring a consultant opinion.
