The Chronic Disease Management Team (CDMT) is an initiative to help improve the quality of care for patients with chronic disease in primary care, focusing on Chronic Obstructive Pulmonary Disease (COPD) and heart failure. The team comprises a GP with Special Interest (GpwSI), nurses, a pharmacist and administrative support.
The overall aim of our service is to work with general practices to assess the current level of care they provide for patients with chronic disease and to develop a plan to help enhance the care. We also aim to help educate and empower patients to manage their disease better.
We aim to work with practice nurses, GPs and other primary care clinicians to offer support in managing patients with chronic disease and in identifying patients who are not yet diagnosed. The support can be tailored to meet the individual practice needs but could include validating chronic disease registers, helping run clinics, reviewing case notes and helping implement case finding strategies.
The Chronic Disease Management Team also offers support and training for Health Care Professionals who work with COPD and heart failure. Education sessions can be arranged at the practice.
