Who we are

 

We are a specialist team of Physiotherapists, Occupational Therapists, Speech and Language Therapists, an Active Case Manger / Nurse, Dietician, Social worker and Stroke Consultant all working together.  The team consists of an Early Supported Discharge Team which supports patients who can be discharged early from hospital with continued therapy intervention and a Complex Team who assess and treat stroke patients with longer term needs.

 

 

How the Team works

 

The overall aim of our service is to provide holistic, specialist, multi-disciplinary assessment, treatment and management for patients over 18 years old who have suffered a stroke. This can occur at home or in an out-patient setting depending which is most appropriate to meet the patient’s needs.

 

 

What do we offer?

 

The treatment programmes involve active treatment, self management, and advice and support for returning to activities of daily living, leisure and work.

 

 

Support to Professionals

 

The Community Stroke Team also offers support and training for Health Care Professionals who work with stroke patients.
