The Falls Service and Community Therapy Team is made up of:
- Occupational therapists
- Falls co-ordinator
- Rehabilitation nurse
- Assistant practitioner
- Active case manager
- Therapy assistants
- Pharmacist
We provide a service for residents of
The overall aim of our service is to:
- provide a timely and responsive service to help maintain people in the community and help prevent avoidable hospital admissions.
-identify patients at risk of falling and those who have fallen and to help prevent/reduce further falls .
The treatment programmes involve:
-Specialist assessment and treatment of fallers/patients at risk of falling in their environment .
-Assessment and treatment of other physical disabilities , with the exception of neurological and palliative conditions
-Falls exercise class - a specialist falls prevention activity , involving exercise, education and advice
-Falls clinic –for unexplained fallers where a multidisciplinary assessment is required.
The team also offers support and training for healthcare professionals who work with people who have fallen or are at risk of falling .
Initial referrals for assessment can be made by:
- GP
- Consultants
- Physiotherapists and occupational therapists
- Social workers
- Disability officers
- District nurses/ACM/practice nurses
- Other Allied health professionals eg. Speech therapists, podiatrists
Referrals should be made on the referral form and must be fully completed.
