Intermediate Care at Home Team

Who we are

A team of Occupational Therapists, Physiotherapists and Therapy Instructors who provide assessment and treatment within your own home when you experience a sudden or recent change to your health and independence as a result of illness or disability. We will see anyone over the age of 18.

How the team works

The overall aim of our service is to promote your confidence and independence so that you can continue, or return, to live in your own home.

What do we offer?

The therapists will carry out an assessment and work with you to identify your goals and the support you need to achieve this.  This will be called your “care plan”.   


The care plan is written down.  It states what you can hope to achieve whilst you receive our service, and how we will help you to do this. 


Where possible you will be encouraged to do things you would normally do at home, such as washing up, dressing and preparing a meal.


The therapists or therapy assistants will visit on a regular basis to carry out assessments, exercises and practice activities of daily living, for example preparing a snack or meal.

Support to professionals

 

The Intermediate Care at Home Team also offers support and advice for Health Care Professionals who work with people requiring intensive rehabilitation due to an acute change in their health and / or functional independence.
