Learning Disabilities Health Services is now known as ‘The Disability Teams’. We are a multi-disciplinary/multi-agency service, located and managed within the Council’s Adult and Community Services Directorate. We work with young people and adults with learning disabilities, from age 16 upwards, in partnership with their families and carers.
The overall aim of our service is to work with individuals and their carers, in a variety of settings, to promote independence, social inclusion, good health and wellbeing. We do this by working with our partners in the Local Authority, Education, Primary and Secondary Care, Housing, Employment, Leisure, Voluntary Sector, etc, in a person centred way.
The treatment programmes involve assessments and interventions from a range of clinicians including: Learning Disability Nurses, Physiotherapists, Occupational Therapists, Speech and Language Therapists, Psychologists, Social Workers and Assistant Practitioners.
Clinicians develop individual and group treatments to encourage skills acquisition. These might include developing strategies to manage complex behaviours, personal care and self-help skills, budgeting, independent travel. Specific pathways have been developed to access audiology screening, health screening for dementia services. Other interventions include epilepsy management, assessment for specialist equipment, individual and group physiotherapy treatments, psychology interventions and programmes to meet communication and swallowing difficulties.
The Disability Teams also offer support and training for Health Care Professionals who work with young people and adults with a learning disability, who may also have associated physical disabilities, mental health problems and sensory needs. Specifically, we also have two posts to support Primary and Secondary Health Services to meet the health needs of people with a learning disability.
All requests for services, including social care assessments, are accessed by contacting the Adults Advice and Assessments Team on 01204 337970 / 71 / 72. Requests are passed on to the relevant Locality or Specialist Team.
