    Neuro-rehabilitation

    The Neuro-Rehabilitation Team aims to provide a comprehensive and co-ordinated range of services to people with an acquired or progressive neurological disorder, enabling people to function as independently as possible. This is achieved by improving or maintaining physical, social and psychological wellbeing.

    Appropriate assessment and treatment regimes are tailored to the needs of each individual. Those with chronic or deteriorating neurological conditions are monitored and ongoing support and treatment is given.

    Advice, education and support to service users, carers and other professional or voluntary services are provided.

    An initial assessment interview is carried out to determine the patient/carer needs, and to ensure that all the relevant specialties are informed and involved.

    We also have a Consultant in Rehabilitation Medicine who will see you at regular intervals if required. The Consultant offers Botulinum Toxin therapy if this will improve the outcome of treatment.

    Referral criteria

    Patients must meet the following criteria:-

    1. Have an acquired or progressive neurological condition.
    2. Registered with a Bolton GP.
    3. Aged 16 years or over.
    4. Medically stable.
    5. Agree to the referral being made and be able to participate in active rehabilitation.

    Referral procedure

    Referrals can be made by:-

    1. GPs, if the patient has a clear neurological diagnosis
    2. Hospital/Community Consultants
    3. Acute Rehabilitation Unit (Hope Hospital)
    4. Intermediate Rehabilitation Unit (Leigh Infirmary)
    5. Stroke Unit (Royal Bolton Hospital)
    6. Other members of the Neuro-Rehabilitation Team
    7. Other Allied Health Professionals following GP/Consultant consent

    If you go into hospital

    You may not be admitted because of your neurological condition but it is important that you alert staff to your needs.
