Urgent Referral and Assessment
Who we are
A multidisciplinary team of nurses, physiotherapists, occupational therapist, support workers and clerical officers.
The referral and assessment team coordinates patients in hospitals and older people from the community into intermediate care beds, nursing beds or assess them for intermediate care at home with crisis support as required. Patients from the community are usually referred from a variety of community sources for example, GP’s, District Nurses and Social Workers. Patients must be over 65, be a resident of Bolton and be medically stable/ or have a diagnosis and treatment plan if referred for Darley Court nursing unit.
How the Team works
The overall aim of our service is:

What do we offer?
Crisis Response Team
The referral and assessment team also incorporates the crisis response team which can provide emergency care support and equipment to keep medically stable patients at home, crisis referrals are responded to within 2 hours . Patients are supported for up to 72 hours (over 7 days a week) while a Social worker assesses and arranges for the Short Term Assessment (STAS) team to take over care support. Referrals are made to appropriate community services for ongoing health and social needs fro example Community Therapist, District Nurses.

Prevent unnecessary admission to emergency short term care.

Provides assessment and rehabilitation in a residential unit for up to 6 weeks (will also assess for future care needs).

Suitable for clients that cannot manage at home but have the potential to return there following intervention.

Client needs to be orientated, able to retain information and not present with any behavioural difficulties.

Team consists of nurses, Physio, OT, Therapy Assistants, Carers, Social Worker and GP cover.

Provides assessment and rehabilitation in the client’s home for up to 6 weeks.

Suitable for clients that can manage at home with assistance from carers up to 4 times a day if necessary.

OT, Physio and Therapy Assistants visit to provide rehabilitation and assessment in the client’s home.
Intermediate Care Nursing Service

Provides a service for patients with an acute illness that will require nursing care for a maximum of 7–10 days.

Team consists of Nurses, Pharmacists, Therapists and Social Workers.

There is GP cover and Consultant Physician support.
