Community Adult Weight Management Team

Who we are

The Adult Weight Management Team is an initiative to help address the concerns and issues around overweight and obesity in adult, primarily over the age of 45. We are a team of professionals who work together to help adult who have requested help or would like help to manage problems linked to being overweight.

The team works with groups and individuals to address the key disciplines that link weight management - including food & nutrition, physical activity and behaviour change and self esteem.

The overall aim of our service is to try and help adults address their weight management issues and achieve/maintain a healthy weight whilst learning to make positive lifestyle choices, to improve long term health.

How we work

We provide support to people who face barriers to healthy eating due to income, budget, knowledge, skills, access to food and facilities.

We use behaviour change techniques and a community development approach to offer practical advice and support on food, health and weight management issues.

We aim to identify and express their own food and health needs and develop confidence in their knowledge and skills to make healthier lifestyle choices

We respond to the needs and wants of individuals and groups by tailoring the activities specifically to them.

Our roles involve being ‘facilitators’, which means we help groups to identify solutions to their own needs. As facilitators we aim to listen and understand, value and learn from people’s experiences and concerns about food.

‘To act as a resource, to provide unbiased, up-to-date and evidence based information on nutrition to colleagues in the PCT, partnership agencies, the public and the media’.

What do we offer?

The Community Adult Weight Management Team provides FREE informal group sessions and one-to-one drop-in sessions to support and encourage individuals towards achieving and maintaining a healthy weight and lifestyle.

Our Group Sessions are between 1-2 hours and are held on a weekly basis over a 12 week period.  The sessions are flexible, practical and informative and moreover provide useful tips to put what you learn into practice and kick off a healthy new you!

We give each group the opportunity to shape their programme and can provide help and information that covers a variety of areas to help you to develop the skills to take control of your weight. These include:

  • Goal setting and managing cravings
  • Recipe ideas including ‘Cook and Taste’ sessions
  • Budgeting and Menu planning
  • Portion sizes, food labels and food hygiene
  • Physical activity and other services available through your local authority

When you have completed the 12 week programme we offer further support should you need it. This can either be through attending another 12 week programme or attending one of our drop-in sessions. Drop-in sessions provide you with one-to-one support when you need it. You can also attend a drop-in session if you can’t attend a group or are waiting for a group to start.

All sessions are held at local health centres and community centres.

Support to Professionals

The Adult Weight Management Team also offers support and training for Health Care Professionals and individuals who work with adults on dealing with sensitive weight management issues.

How do you refer?

We welcome referrals from individuals, health trainers, doctors, nurses and other people working with adults with concerns about weight.

We are mainly working adults who are over the age of 45 and have a BMI between 25 - 29.9, although no-one will be turned away. We will also accept telephone enquiries direct from health professionals and individuals.
