Intermediate Care - Residential
How the Team works
The overall aim of our service is to prevent avoidable hospital admission and or prevent admission to long term residential care whilst maintaining individual independence and wellbeing and supporting people to remain living in their own homes.
Enhanced nursing beds offer treatment of acute conditions that can safely be delivered in a non hospital environment that maintains independence and confidence.
Care is holistic, patient centered and delivered by a highly skilled multi disciplinary workforce.
What do we offer?
The treatment programmes involve a comprehensive assessment and rehabilitation service facilitating efficient and swift discharge from hospital where a period of assessment, rehabilitation and recuperation will enable the patient to regain independence.
The enhanced nursing beds delivers nursing and medical care to acutely ill patients alongside comprehensive assessment and rehabilitation.
Support to Professionals
The IMC Team also offers support and training for Health and Social Care Professionals who work with IMC.
How to access the service
A referral to IMC services will be on an open professional referral basis via a single entry point. Referrals are accepted 8am-7pm Monday to Friday and 8am-5pm weekends and Bank Holidays by telephoning 01204 390286.
Admissions to the acute beds are via Bolton Community Unit.
