Who we are:

Our service leader, Kathryn Morton, heads a team of Registered Dietitians, Assistant Practitioners, Dietary Assistants, Food and Health Advisors, and Community Nutrition Workers. We work together to provide a quality service to residents in Bolton who need Nutritional Support including Home Enteral Tube feeding; General Dietetics focusing on Diabetes, Obesity, and Coronary Heart Disease; and Adult Weight Management. The service is delivered in a method most appropriate to the client as described below.

 

How the service works:

The service provides nutrition and dietetic care to adults requiring dietetic intervention based on individual needs. Once a client is referred into the service they are triaged to an appropriate care pathway for assessment, treatment, and monitoring.

 

What does the service offer?

We offer appointments giving support, advice, and education to clients referred into the service. We do this in a variety of ways to suit the client including: group sessions at health centres; one to one consultations in clinics, nursing/residential homes and intermediate care; and in clients’ own homes. We hold clinic appointments at most major health centres in Bolton including:

·        Crompton

·        Farnworth

·        Great Lever

·        Little Lever

·        Pikes Lane

·        Westhoughton

·        Horwich

At present clinic appointments for Nutritional Support clients are held only at Crompton Health Centre.

 

Support to Professionals

We are experienced trainers for student dietitians currently providing C placement training. We have strong links with the University of Chester.

We also offer support, education, and training for health care professionals in Bolton.

 

How to access the service

We accept referrals from all health care professionals using our specific Nutrition and Dietetic referral cards. If you are a health professional and you require referral cards please contact us.
