Our service leader, Kathryn Morton, heads a team of Registered Dietitians, Assistant Practitioners, Dietary Assistants, Food and Health Advisors, and Community Nutrition Workers. We work together to provide a quality service to residents in Bolton who need Nutritional Support including Home Enteral Tube feeding; General Dietetics focusing on Diabetes, Obesity, and Coronary Heart Disease; and Adult Weight Management. The service is delivered in a method most appropriate to the client as described below.
The service provides nutrition and dietetic care to adults requiring dietetic intervention based on individual needs. Once a client is referred into the service they are triaged to an appropriate care pathway for assessment, treatment, and monitoring.
We offer appointments giving support, advice, and education to clients referred into the service. We do this in a variety of ways to suit the client including: group sessions at health centres; one to one consultations in clinics, nursing/residential homes and intermediate care; and in clients’ own homes.
